Position Description

Job Summary 
The Registered Nurse I is responsible for guiding and supervising assigned nursing personnel in delivering patient care. The RN I will collaborate with nurses, physicians, and other health care professionals in the delivery of patient care. Assume responsibility for management of a selected case load of patients as defined by the clinical practice area.

Essential Duties and Responsibilities

  • Performs assessments and data collection in an ongoing and systemic manner, focusing on physiological, psychological, psychosocial and cognitive status
  • Responsible for performing face to face and phone triage. Assesses patients’ medical urgency and determine care needs based upon presented symptoms and department standards of care
  • Assesses and document conditions, wound status, cardiopulmonary status, neurological status, skin integrity, and vital signs. Ensure patient's rights are upheld as by the compliance committee and officer
  • Accurately recognizes abnormal diagnostic values for the adult medical patient. Interprets their significance and follows through with appropriate Physician
  • Formulates a patient specific goal directed plan of care which is prioritized and based on nursing assessment and patient outcomes
  • Develops, individualizes, and professionally modifies care through physical, psychosocial and cognitive realms of nursing assessment and interventions actively considered
  • Establishes measurable patient-centered outcomes with patient, family and other disciplines. Maintains confidentiality by communicating patient/family/significant other information only to health care workers involved in the care of the patient in an appropriate manner
  • Ensures that total patient care needs are met by following established multi disciplinary plan of care. Implements care in a knowledgeable, skillful, consistent and holistic manner
  • Explains rationales for all nursing, technical and therapeutic interventions selected and conducted.  Demonstrates knowledge of the relationship of pathophysiology to procedures and interventions
  • Seeks participation of patient and significant others in care. Contacts patient for appropriate follow-up
  • Performs technical skills, seeking assistance as needed, according to established procedures, protocols and scope of nursing practice care standards
  • Utilizes knowledge of dosage calculations, indications, actions, routes, and side effects for proper administration of medications. Obtains appropriate consents before administration of vaccines/other medications as appropriate
  • Exhibits an understanding of infection control principles and universal precautions when changing sterile dressings, injections, or performing any procedure that required sterile techniques
  • Evaluates and documents the effectiveness of Nursing intervention and teaching by patient’s response to teaching measures, behavior outcomes, knowledge base/understanding and/or skill demonstration and ongoing reinforcement of educational process
  • Performs complete documentation responsibilities relating to patient's medical records in a timely, accurate and concise manner
  • Demonstrates awareness of legal issues in all aspects of patient care and department function and strives to manage situations in a reduced risk manner
  • Attends mandatory monthly Nursing supervisory meetings and occasional weekend trainings as required
  • Performs all other duties as required

Qualifications

Education:

NLN Graduate from an accredited school. Must have a current Connecticut State Licensure. Current American Heart Association Basic Life Support Certification preferred.

Experience:

Minimum of one (1) year experience in a long-term care facility or rehabilitation center. Clinic experience is highly desirable.

Knowledge/Abilities:

Excellent Patient Management skills. Must be strongly motivated and able to accept supervision. Computer skills are essential with the ability to learn AS400, SRS, MD Link and Mednexus. Ability to provide phone triage along with knowledge of referral process, medication refill procedures and Hollister charting are also required. Knowledge of CLIA waived lab testing, Urine Dipsticks and Gucometers are necessary. Excellent leadership qualities and knowledge of management skills are necessary. Bilingual individual is desirable.

While performing the duties of this job, the employee is responsible for bending, lifting, walking, carrying or pulling objects. The employee must occasionally lift and/or move up to 25 pounds.

The job requires exposure working with body fluid and hazardous chemicals. When under these working conditions, OSHA standards and universal precautions must be followed at all times.
